What You'll Do:

As a R.N. Assessment Coordinator , you will carry out our Mission of Love by completing resident assessments and completion of all associated paperwork required by state and federal regulations governing the resident assessment process. Our R.N. Assessment Coordinator are responsible for the following:

  • Schedules RAI assessments in a timely manner.  Using the computer system, communicates the schedule and any other necessary information to interdisciplinary team members.  Creates initial care plan meeting agenda for staff to schedule family meetings.
  • Strives to complete and data enter into the computer all required resident assessments and tracking forms required by regulations in a timely fashion, electronically or manually sign; print and place in appropriate medical record.  Reports completion or signature non-compliance to the appropriate supervisor.
  • Creates and updates resident specific care plans in the computer and hard copy as applicable.  Print and place with the appropriate record.  
  • Is familiar with all current state and federal regulations related to resident assessment and care planning process and assumes responsibility for maintaining current knowledge and skills with respect to assessment process, state and federal regulations, reimbursements related to the RAI process, and technology.  Maintains knowledge of facility procedures and nursing process.
  • Assists in developing and updating of procedures related to the resident assessment process; participates in quality assurance programs, assists in staff education related to documentation and RAI process; participates in Interdisciplinary Care Plan meetings when necessary.
  • Establishes and maintains collaborative communications with residents, their responsible parties, the Director of Nursing, other health care professionals and facility departments.

What You Definitely Have:

  • Graduate of an accredited school of nursing required (Registered Nurse). Bachelor's degree preferred.
  • Current license as a Registered Nurse by the PA State Board of Nursing (required).
  • 2 Years geriatric experience (preferred).
  • CPR certification (prior to or upon employment).
  • Previous experience in performing nursing assessments (required).
  • Current knowledge of state and federal regulations pertaining to long term care facilities (preferred).
  • Basic computer knowledge and typing skills (preferred).
  • Attention to detail and high degree to accuracy (required).
